Non-receipt of provisional pass certificate and mark sheet

I appeared for exam (exam code 730, Register No: 010M8115P07066) in May the only exam of B.Sc to be cleared which i cleared in this attempt. Results were declared in October but it is early February. I am not in receipt of the provisional certificate and mark sheet. I am in urgent need of these two. I beg the department to give me a response. I have tried reaching through phone but of no use. I would be very grateful if u could at least assist me in letting me what should i do in order to get my degree certificates and mark sheets.
