Incorrect name & delay in issuing the passport

Dear sir,

This is to acknowledge you that I've applied for passport on 17/11 & File Reference Number: PAT-A125737-09 .

i don't know how it happened my name is not displaying correct actually my name is Vikash Kumar Pandey but whenever i check status of my passport it's shows only Vikash Kumar

so it's my kind request to you pls correct my name as mentioned above & execute the passport as early as possible.

pls do the needful & let us know the status.
