Regarding delay in Passport

Sir,

I applied for my passport on 26/10/09 in Bhopal Passport Office.

My Application No. is z292457.

Since last one month the site displayed a status "Due to some circumstances, your passport is delayed and will be dispatched in a week's time ."

But Now its giving another message, which says "Please furnish a fresh application form with all the columns filled up, all declarations signed and photographs affixed in the boxes given in the form. "

Now what should i do??
