Respected Sir,

PASSPORT NO: H3414422. (ISSUE DATE: 26 oct. 2009)

With due respect i would like to mention that my name is SAMSUZ JAMAN CHOWDHURY. But my name has been Mis-Printed in Passport as 'SAMSUZ ZAMAN CHOWDHURY', Only the letter 'J' of MY Middle name is being printed as 'Z'.

Sir, to my full knowledge i know that there has been no mistake in filling up the form. So i would be humbly grateful if you would kindly rectify my Name as SAMSUZ JAMAN CHOWDHURY as I had to travel to Dubai & Russia in the first week of June Month.
