Passport not received

Dear Sir,

I Shirish Jain, my passport file no. BPLA-011409 09 dt. 01/05 my police verification already done. my passport not issused till date. One letter received from passport office dt. 26/10 received on 18/11 for reason "The name given in passport application dose not telly with the name given in your school certificate." I am also reply against this letter on 20/11 through speed post. but till date not received any quiery from passport office bhopal. So please issue my passport as early as possiable.
