Egarding passport form

Respected Sir,

Myself Mohammed Mohsin s/o Shabbir Ahmed, I'm from Sikar (Raj.). My problem is that today i applied for passport and in my form by mistake i wrote address "Mohalla karigran, ward no.20" although it is also right but in my other documents my address is "Rana ka jaanv, ward no. 20 Sikar"

I request you to make correction in my form...
