Delay issuing in passport

Sir,

I myself Prashant kumar applied for a passport, dated on 05-03 bearing File Ref. No.-W370599-11. In regarding this i had submitted the following documents which are, (given, police verification completed)

1. Residential Certificate issued by S.D.O Muzaffarpur &for Add. proof

2. Pan card along with letter that already contain residence add.

3. Learning driving license

4. Matriculation certificate &-for Age Proof

5. 10 2 Certificate

After furnishing all the regarding information, there still needed the furnish the required information within 15 days. When is checked my application status.

so, i have a humble request to please look into this matter and resolve my problem so that i could get my passport on due time. I shall be highly obliged to you for this kind act.
