Sim card rejected

Have mistakenly with out my knowledge on what exactly the pin Number and the PUK number were meant for. I Entered wrong info as i wasn't aware of what it was nor any information is given to the user (In this case Me)..

Now the SIM is rejected and I need my SIM to be immediately activated.

Give me information and Make it activated ASAP. My mobile Number is 9425055862
