I have lost my Sim card no (don't know) phone no 9940375600 purchased from Basullah Road, T. Nagar, Chennai 3 months back.
I lost cell phone along with above said no sim while driving vehicle on 30 Apr'10 and could not inform you immediately because I was on the way to go to Central station for going to Hyderabad, now I am at Hyderabad. As soon as I reach Chennai, I will search for other information for feed back to you.
Once again I request you the kindly block the sim card to avoid touse misusing of my No.
I am sorry to say this is 2nd time I lost my cell phone. I request you to kindly block the sim card and arrange to issue me the same no.

Kindly intimate me after blocking sim card.
