Issue the mob sim without verification

Dear Sir/madam,

Somebody got issued mobile no. 7742621412 in my name and abusing to other persons. A person came to my home and says " he is getting call from this no. and it belongs to you and your address" i surprised that i never applied for this no. and how it happened. Then i blocked this sim card from airtel. but i want to know how any person can issue the sim from my name and address.

If somebody has theft my address but never can theft my image so i just want to know who about that image.

so plz tell me how can i know about that person name and address.
