Wrong mother name printed in passport

Hello,

This is regarding the wrong name printed in my father’s passport. We had applied for the New passport. Ref No. is: SUR-B01409510.

We have also check and verify the forms before applying there was no mistake in filling form but When we have received the passport (J3917964) Mother’s name and Wife’s Name is same. Mother’s name is wrongly printed.

When my father went to passport office surat, and met Mr. M C. Kadva he told now again you will have to apply for passport.

My question is why we waste our money and time again when we had provided the proper information? And is there any guarantee that they will not make the same mistake again. Should I ask information (scan copy of my form) in RTI?

We need correct Passport as there is no mistake in filling the form.
