Tax refund

Hi I have received Rs. 4610 in the month of Feb to my old address and the moment i received the cheque I had 3 days to clear that through bank.

But There was delay in the processing from bank side and I could not get my refund. The cheque is with me now and just wanted to know where should I return it so that they will issue me a new cheque/DD

I could see the branch name as - S>B>I Ghatkopar (W) Mumbai.

Please help me in getting the correct address.
