No full and final no P.F

Respected Sir/Madam,

My name is Sidhath Jain and my employ code-CS15332. i was working in HDFC Bank (Mall road Ludhiana branch) as a sale executive from 11- DEC- 2007 to 20- may. I left the job due to some personal reason and my resignation letter was accepted by my ser.. Mr. Gulshan arora at that time on 22-05. i haven't got my full and final P.F from the bank. I asked my ser. to release my full and final and give me my P.F number bt he refused to give me all the information. please help me in this matter so i get my P.F money and salary,
