Statement of credit card

I m holder of cc 4050282002049042 . There was a payment delay and the cc was blocked.i m serving abroad and deposited a cheque of the principal amount 20 days back.. want to know whether the card is activated or not and my monthly statement is reqd to be sent on to my email.
