PF Withdrwal status

Sir,

I was with HDFC Life during 2009 as Business Development Manager in Agency channel. I have submitted my PF Request form in our HR on January. As per the system it is accepted in their HO.But till date the same has not been credited. Kindly help for finding the status of it. My emplyee id was-31204.Hoping the same.
