I am holding credit card No.4384591393348673 by which you had contacted me and requested me to join for the membership of HealthCheck programme connected with PLAT5. But your official phone numbers quoted are withdrawn and no way to contact with the membership programme. The promised duration after receiving the the terms & conditions there was no response from your side except some SMS to contact you by me through internet for any healthcheckup which I do not trust. The membership fees, Rs.4853/-has al-ready been debited from myabove card and paid the same promptly. But from your side no response. When I contracted through HSBC customer care, I was was told the above programme has been withdrawn I was asked to send this letter informing my cancellation of this membership and hence I am writing this letter, so that I will get reimbursed my money. Please cancel the membership fees and reimburse me the fees. Regards,
