Dear Sir,

I am possessing your Credit card No. 4183 5699 0000 8090 which is expiring in May.

Now, I don, t want to use the card. Hence, I request you to kindly, cancel my above card at the earliest.

I would also like to inform you that I have change my residence from Ahmedabd to Mumbai at following address.

A-1/B-003, LIC Quarters, Kanderpada Circle, Dahisar West, Mumbai - 400068.

Kinldy do not make any corespondance at my Ahmedabad address.
