NON-ISSUANCE OF PREMIUM RECEIPT

Dear Divisional Mgr of LIC,

I had purchased Jeevan Saral Policy from Mumbai Branch No.907, wallace street, fort, mumbai -1.

My Premium mode is through ECS facility from my savings a/c of Rs.1000/- which is like a SIP Plan.

Now, I require a Premium certificate statement for my tax declaration purpose from April`2010 till Dec`2010. My Agent is outstation. I called the call-centre informing them of the same but no response received.
