LICI/Health Plus Policy
Non-receipt of Health Card under Health Plus policy of Life Insurrance Corporation of India (LICI)

I am a policy holder of LICI and availed the Health Plus policy (Policy No- 579185894) of LIC about one half year back. I have already paid the three premiums (of Rs. 2500.00 half yearly) under that policy.

But I am very sorry to inform that, I have not yet received any Health Card under this policy.

It is, requested that I cann get the said Health Card as early as possible.
