Payment made not reflecting

With reference to LIC policy no. 113336597, There was a debit entry of INR 13, 632 (thirteen thousand six hundered thirty two) in my credit card stement of ICICI Bank bearing no. 4477463528306026 in the month of Nov. 2010 (around 14/11), despite the fact that I have already made the payment in cash one week earlier at - collecting center A001 (Lata jain) vide transaction no. 82463494 dtd 08/11.

pls look into the matter as double payment made to LIC but not reflecting
