I have invested Rs.25, 476/- in new york life insurance through Axis Bank during the Month of November '10, Kakinada and Policy issuued against my investment vide no.827405341. Inspite of my repeated reminders the Axis Bank, Main Road, Kakinada as well as the Max New York Life Insurane Agent who is sitting at Axis Bank, Kakinada are not responding.

I am unable to get the premium receipt and policy as on date i.e., 19.01.2011. Kindly help in this matter.
