Complaint on Lost of PAN Card and Request for Duplicate PAN Card

Dear Sir,

My name is Nagesh Narayana PAN Card no. ABDPN2941B. I have lost my PAN card while in the office at IB Times, Ground Floor, Matten Tower, Diamond District, Old Airport Road, Bangalore-560008. Kindly register a complaint against it so that no one can misuse it. Besides it I would request you to issue me a duplicate copy of that to the below mentioned new address.
