Atm card lost and unfair use of atm card

RESPECTED SIR,

I am a/c no 4167000400329574 of your bank branched at Hisar cantt. My atm card no is 5126520067031810 . my atm card was lost when i was travelling from rohtak to hisar by train on 3 july at about 4 pm. the atm card was misused by some person and it came to our notice that it was shopped at different places. the details of shopping by my atm card is as follows :-
