The above said policy was matured with effect from 28.3.2010 (vesting date) on receipt of option form vide ref no-IPP/19J/Annuity Option/1470000002 same was send to Sr/Br Manager LIC Of India Chirwa Rajkal complex main market ChirawaPin -333026 dated 31/3 by Exercising the following option.
1) Pension option
a) Annuity For Life of the Annuitant
Commutation Option
Option -1 I desire to receive 25% of Notional Cash Option and annuity on the balance NCO as per selected pension option
3) Option for mode of pension payment- Quarterly
I have received 12 nos of PDC at my Jamshedpur address ie. 65, New Baradwari, Sackhi Jamshedpur 831001 without 25% commited amount as per exercise option.
I had thereafter requested vide my letter dated 15/5 to Sr Branch Manager of LIC Of India Chirawa with a request to send the 25% commited amount to my present address at kolkata ie. 7S, Cornfield Road Ballugunge Kolkata-700019 and
also requested to send the amount to my saving account no ICICI 00341525002 (Ballugunge branch) Kolkata-700019. I had already made an online complain which has been duly registered by giving 15 reward points but without any result.
