No Information about Policy Cheque cleared on 13/04/1o

I had approached Policy Bazar for my health Insurance. On advise of Ms Puja appropriate forms and a cheque for Rs 4002 (No.040986 drawn on HDFC Bank) was sent which was cleared on 13/04 and medical examination was done within next 3 or 4 days.but till date I have not received the policy or any confirmation about the acceptance of proposal. you are requested to look into the matter and inform me about my Insurance
