Non issue of extended warranty book

I'm Ajay Naik. We have purchased Chevrolet Spark in August. from Ashtavinayak Auto, Vasai. no. of car is MH-04 DY 6124. company has offered us two years of extended warranty on additional payment of 5000/-Rps. the dealer was supposed to give booklet of that extended warranty. but still they haven't issued it. we have called them many times. as we don't have that extended warranty no. we had to pay the service charge 1000/- Rps. which we are not supposed to pay.
