Dear sir

i had renewed my comprehensive health cover policy on 16/may, in which you have wrongly mention DOB and name of my wife .this complain was given to custmorcare deprtment but since then there has been no action from your side and nobody seems to bother about it.i still haven't recieved my health policy [cashless] cards.it's been 5 months, i'm with a paper policy with the wrong name and DOB mentioned in it.in case of any emergency.what am i supposed to do and who should be held responsible for this? it's been a bad experience with you. i will think twice before renewing it next year.
