ICICI Pru Health Saver policy
Issue of ICICI Prudential Health saver policy

I have applied for health saver policy vide application No: HS02157532 for health insurance for Rs 5.00 lakhs and paid the premium amount Rs 25, 000/- vide check /D.D No 000478 Dt 17.02.2010 . I have completed all the Medical Examination Reports (MER) in the referal diagnostic centers as mentioned byyou. Till to date i have not received any policybond or health card. kindly issue orders to send the policy bond and health card.
