Lic health protection plus policy
Identity cards are not received until today

Dear sir,

i wish to bring to your kind notice that i have taken a health insurance policy from lic of india, product name "HEALTH PROTECTION PLUS" on 30/11, against policy no.844527589 in favour of MR. DASARI. RAMAKRISHNA PRASAD.Inthis connection we have not received health insurance policy identity cards from ''FAMILY HEALTH T.P.A., PVT., LTD., So i request you please send a identity cards to me.
