Account Debited but amount not came from ATM-Kolkata Sector V

Dear Sir,

I am holder Debit Card of SBI, on date 11-08-09 at 19:06 while withdrawing amount of Rs. 1000 from ATM located EE12, Sector 2, Salt lake City, Kolkata, the transaction declined and amount doesn't came and at the same time my account is debited by the same amount.

Here I am providing some detail of my withdrawal Slip:
