Not received correct pan card

Dear sir/madam

I applied for pan card in the starting of 2008 and I got the pan card as name yogesh kumar saraswat

father's name yogesh kumar sarsawat

and i reapplied for new pan card and i submitted the pan card afther that i didn't get the new pan card,

pls tell me how can get the correct pan card, as soon as possible
