I have misplaced blue book (customer card). approached to dealer for issuance of another blue book

He refused to issue and advise for lodgement of FIR with police authority.

Please advise me the right procedure and also instruct dealer for issuance of same.

I also contacted marketing officer of INDANE she also endorsed the view of dealer
