Non Receipt of Election ID Card

Dear Sir,

I have applied for Election Card for 4 times by now including orally to the election commission's personal who visited at my residence, and still am waiting for my card. And even i have given in the record of Enumeration bearing the Sl. No. 2325314 and Sl.no. in Preliminary List is 491, with the name "Indu B Pandya".

Kindly let me know what is the status of my Election ID Card, if its not issued yet, kindly let me know for new application.
