Annual fee for Life time free card, Renewal of expired card

1. My card 4476 9299 9756 6333 expired on 1.1.09 - New card is not recieved so far - Pleases issue a new card

2. I was told that mine is lifetime free card - you have sent statement for Rs. 1261.51 towards annual fee - pl. waive it
