Re-issue for new card

Dear Sir,

My name is Vivek Badoni (on card Vivek Vadoni), i have lost my card before 2-3 months ago, then I have applied two times for new one. I have received the statement for new card but still iam waiting for card

My card no is-4384 5913 9278 7046 & account no is-4384591392787335.
