ICICI lumbard insurance on Credit card

With out my knowledge and confirmation ICICI bank paying amount to ICICI lumbard on behalf of me and putting in to EMI.

If I call ICICI lombard not reponding properly and showting on me. For cancellation and all unessarly asking 75% of the total amount.
