Account Opening Form

ICICI Direct has approached me again to submit my account opening form, failing to do so they have said that they will de-activate my trading and DMAT account. This is the second time they are asking me to re-submit the account opening form in the last 4 years. They keep loosing my details and ask me to sign the forms and submit all the proofs again. I am tired of doing this one, i want SEBI to take strong action against them.
