I ashoka jain f/m jaipur purchased a ICICI invest shield cash back policy but i surprised and shock when i see my policy document

1- my photo on policy was other photo

2- date of birth 20 years less

3- nominee name wrong

i contact to icici offoce but no responce/ no refund of money / no solution
