Premium payment Receipt not received

Dear sir,

have paid the policy cotract No. 05146949 premium for the year 09 on 17/03 by cheque drawn on Oriental bank of Commerce, M, G Road Bangalore. it's almost 3 months now & have not received the receipt for the same till date (9/6)

Request you to kindly look forward on this matter and send the receipt as early as possible.
