Dear Sir / Madam,

I had lost all my policy papers in shifting in the year 2002 which was for my daughter Simran Tandon, Date of birth 26th december 1996. I was totally unaware of this site untill last month when I updated my other LIC policy on the net. I do not remember the account no. and I do not have any photostat copy of the documents.

The details I can provide are as follows- Name of the Policy- Jeevan Surbhi / Jeevan suraksha

Name of the policy holder- SimranTandon

D.O.B - 26th december 1996

Father's name - Sunil Tandon

D.O.B - 20th April 1971

Address- K.G.- 2 / 122, Vikaspuri, New Delhi- 110018 [earlier address ]

Premium per annum - 25, 000 Rs.

Term of payment - 1997 to 2002

amount of money deposited - 1.25 lakhs - 1.5 lakhs

I have another policy Jeevan Varsha no. 286529852 started in varanasi on 28th March.

My appeal to the authorities concerned is that please enable me to obtain a Duplicate Policy as soon as possible so that I can restart my policy again.[earlier policy]
