Dear Sir/Madam,

This is in reference to my policy plan is JIBAN SARAL, It was registered in the month of Nov from your 31C branch in Connaught Place, New Delhi. I have paid 3000.00 as its 1st premium for one yearly. (yearly plan).

As I have misplaced its receipt, therefore I request you to please send me or mail me the duplicate recipt with all other policy document. Till date I didn't recive any policy papers also.

Please consider my application & do the veedful at earliest.
