Not given payment

My father was government employee in GEBin 1999 insurance deduct from salary he was died on duty but LIC cannot give us claim amount because they lost docket and no budy can give proper responce from LIC so, it is nearly 10 years ago please send information on my mail address [email protected] so if possible where i have to complain give me address and phone numbe
