Amount of PF not received

Sir/Madam,

My PF no. is HR/8645/7953, I submitted my PF form on Apr.30 for credit in Axix Bank A/c No. - 056010100651608 and i was told that i would get the amount within 3 months, however I haven't get of anything about the same till date.

I would request you to kindly look into the matter and do the needful ASAP.
