I have taken a policy No. 100300/47/01/9600022 / 03/96/30406 Period of cover from: 23/02 To 22/02. The name written in the policy is VIVAS BHANDARI instead of VIKAS BHANDARI. I have raised a paper about One and Half year before through my agent Mr. Goyal to change my name in the policy. I am continuously following up with him regarding my policy, as I have given original copy of policy with that NAME CHANGE FORMAT and a photocopy is with me. Mr. Goyal (Agent) told me that he has left this business, but he is following up for my policy. But still nothing has happended till date. I request you to kindly look into the matter, so that I can get back my policy paper with changed name. I shall be thankful to you.
