I have submitted duly filled and verified application form along with all supporting documents, photographs, etc required to be submitted for issue of photo-PAN Card on 5th January (which was not sent to me along with PAN Number intimation). I already possess a PAN card without photo. Since then I have neither received PAN Card nor heard anything from the concerned. It has been close to 2.5 years now.

Documents submitted – Copy Of PAN allotted, photographs, etc.
Application Submitted at – UTI Technologies Services, Gulab Bhavan, New Delhi.
On enquiry at UTI, I was told to submit an undertaking regarding spellings of father’s name which I submitted on 19th February.
I now request that PAN Card may please be issued to me which I could not get along with PAN Number allotted to me in the year 1997. I am a regular Income Tax Payer and submitting my I.T.R’s regularly.
