i have lost my Pan card number BFJPS8149P

How to get a new/duplicate PAN Card and what are the exact procedures.

and also my PAN card have mistake of name my sir name is in place of first name nd first name is in place of sir name please tel me whole procedure of applying the new and correct PAN card

I have filed FIR also

And after filling FIR where am I supposed to submit it and do I need to pay any fees for the duplicate PAN. If so, how much is the fee for it?
