Lost my PAN Card. Wants to reissue of the same

Hi All

Please be informed that I have lost my PAN Card few days back. My PAN Card No. is AJYPB2859F.

Can anybody please suggest me what all I have to do to get the PAN Card reissued with same PAN Number.

Your prompt revert will really help me and will really be appreciable.
