I, Ghanshyam Arjun Hemade, Filled a application form (Acknowledge No.027253521) for the PAN card on May 28 at Hotel Royal Central Orchid, Manipal Center in Bangalore. On that day we had our "Induction Day" programme from the company "MPHASIS BPO Ltd.". We filled the application form for PAN card because our company asked us to do so. A man on the behalf of the respective department collected our application form & amount Rs.67 But I didn't received the PAN card yet. For communication purpose I gave the Address of my office ie,
