Incorrect billing & not responding to request

I had applied twice to close the telephone no. 9818187542 in november & 15th december but despite my repeated request the company is not showing any interest to close my no and adding on to my bill.

every time customer care executive pretend to motivate me not to close the no. But to make his points he enters the comments in his favour, that he had satisfied me with his plans and motivated me not to close the no. This is utter nonsense and i am being harrassed in this note. This is my request that kindly close the no.immediately and penalise those who are guilty or i will be compelled to go to the consumer court. help from your side is highly admissible.
