Apollo DKV Insurance Co
Not received the Medical Insurance Document

Dear Sir/Madam,

I am Resham Giri & working with M/s Polygel Technologies (i) Pvt. Ltd. in Delhi Branch.

I purchases a Medical Insurance Policy with Apolly DKV Insurance Co. Dated As per receipt No.1100339575Dated:16.07.2009 also paid a premium Rs.3261/- (Three Thousand Two Hundred Sixty One.only)

But till date:20.08.2009 I am not received any document paper from Apolly DKV Insurance Co. iam lot of time talk to customer care extenuative but the are told me document send the dated:11/08 but till date I have not received the document.

Kindly do the needfull.
